in the care of your pre-surgical patient.
Our goal is to communicate clearly and efficiently to the referring provider regarding testing results from your patient’s visit. Our office will send medical and pain clearances to the referring provider within 48 hours of the patient’s appointment at The PREOP Center as well as any clinical or diagnostic testing results from this visit.
If you would like to refer a patient over to The PREOP Center, please fax over the patient referral form as well as a patient demographic sheet and last clinical note. Please include any diagnostic testing results that our providers may need. You may download our referral form below.
